Specialized Patient- Centered Home Healthcare Services

Our home healthcare services includes chronic disease management and evaluation of a patient care nursing plan, observation and assessment of patient’s progress condition, daily vital signs assessment, administration of medications as prescribed by physician and instruction on medication such as (Injections, IV’s, inhalation, oral, infusions, rectal, and enterals), management of palliative care patients with end stage of cancer or advanced progressive illness/terminal life threatening disease of disabilities including pain management, daily insulin injections, supervision of patients with diabetes and monitoring blood glucose, nasogastric tube feeding management, nasopharyngeal and tracheostomy aspiration, indwelling catheter care and maintenance, wound care and bedsore management, ostomy care and maintenance, point of care testing such as blood glucose level, dipstick urine, venipuncture, naso-gastric tube care and maintenance, Foleys catheter insertion, pain management, continence nursing assessment and management, patient care education and counseling, consistent with the type/nature of therapy and/or care management needs, assessment for oxygen therapy requirements

Our Home Healthcare Services

Home Personal Care : Our trained local Patient Care Assistants assist with you and your loved one’s activities of daily living (ADLs) for safe and independent living at home. This includes eating, bathing, getting dressed, toileting, transferring and continence, and timely oral medication, vitals monitoring.

Home Nursing: Our nursing assistants, registered nurses can provide you for your loved ones simple nursing procedures like wound dressing, urinary catheter change, ryles tube, nebulization, bedsore care, stoma care,giving IM, IV medications, facilitating coordinating with your primary physician at clinic and hospital in the comfort of your home.

Home Therapy: Our home therapists are here to support your rehabilitation and recovery efforts when you are discharged from hospital to home, through personalised exercise and therapy treatment sessions which includes Physiotherapy, Speech Therapy, Occupational Therapy, Yoga Therapy.

Home Medical: Our team of doctors are here to provide your teleconsultation, home visits consultation for your loved one is sick or discomfort or feeling unwell at home and unable to visit a clinic or hospital, our registered doctors are here to respond to help patients at home.

Medical Equipment: We provide homecare medical equipment like BiPAP, CPAP, Suction apparatus, Oxygen concentrators, Patient Cot on rental basis. Home delivery of patient care consumables like diapers, underpad, gloves.
