Appointment Booking: A convenient date and time are scheduled for a comprehensive in-home assessment, either virtually or in person, based on client preference and urgency. Confirmation and reminders are sent.
Client Outreach: The client (or their family/representative) reaches out via phone, website, or referral.
Needs Assessment (Initial): A friendly care coordinator gathers basic information about the client’s needs, current situation, and what they are looking for in terms of care. This helps determine if home care is the right fit.
Information Sharing: The coordinator provides an overview of services, answers initial questions, and explains the next steps.

Step 2: Comprehensive In-Home Assessment & Personalized Care Plan Development

Review & Approval: The proposed care plan is presented to the client and family for review, feedback, and final approval, ensuring it meets their expectations and needs.
Detailed Assessment: A registered nurse or care manager conducts a thorough in-home assessment. This includes evaluating physical, cognitive, and emotional health, daily routines, home environment safety, and family support systems.
Client & Family Collaboration: Open discussions are held with the client and their family to understand their goals, preferences, cultural considerations, and any specific concerns.
Care Plan Drafting: Based on the assessment, a personalized care plan is drafted, outlining specific services (e.g., personal care, medication reminders, meal preparation, companionship), frequency, duration, and desired outcomes.

Step 3: Caregiver Matching & Service Commencement
This step focuses on selecting the right caregiver and smoothly initiating care services.
Care Commencement: Services officially begin according to the agreed-upon schedule, with the caregiver providing compassionate and professional support.
Caregiver Matching: A suitable caregiver is carefully selected based on the client’s needs, personality, interests, and any specific language or cultural requirements. The company ensures the caregiver is fully vetted, trained, and qualified.
Caregiver Introduction: The chosen caregiver is introduced to the client and family, often with the care manager present, to facilitate a comfortable transition and ensure a good rapport.
